Job description
Gentiva seeks an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours. You will help ensure continuity of care and strong clinical management for patients and families during nights, weekends, and on-call hours.
Responsibilities include assessing needs, coordinating with the IDG, updating the plan of care with physicians, educating families, and maintaining accurate documentation while upholding infection control and regulatory compliance.
