Job description
Gentiva, SouthernCare's parent company, seeks an Afterhours RN Care Manager to ensure high-quality hospice care during nights, weekends, and on-call hours. You will provide direct patient care, assess needs, and coordinate with the Interdisciplinary Group to uphold the plan of care.
You will educate families, document thoroughly, and support regulatory readiness while leading field staff with a compassionate, professional approach.
