Afterhours Hospice RN Care Manager

Job description

SouthernCare, part of Gentiva, is seeking an Afterhours RN Care Manager to provide compassionate hospice care outside standard hours, including nights, weekends, and on-call coverage. You will assess and coordinate patient and family needs, provide direct in-person care during after-hours shifts, and maintain clinical oversight, collaborating with the IDG to ensure seamless care and regulatory compliance.
