Job description
Heartland Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care during nights, weekends, and on-call hours. The role includes direct patient care, assessments, care planning, and collaboration with the IDG to ensure seamless coverage and high-quality outcomes.
The ideal candidate is an RN licensed in the employing state with at least one year of hospice or related clinical experience, and strong communication and leadership skills.
