Job description
Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours, including nights and weekends. You will assess needs, coordinate care, and maintain clinical oversight for assigned patients while supporting the Interdisciplinary Group (IDG) and ensuring compliant documentation.
The role requires RN licensure, 1+ year in relevant settings, and strong communication and leadership.
