Job description
SouthernCare New Beacon Hospice (part of Gentiva) is seeking an Afterhours RN Care Manager to provide essential hospice care during nights, weekends, and on-call hours. You will assess patients, coordinate care with the IDG, and ensure timely documentation and regulatory compliance.
Responsibilities include direct care, plan-of-care updates, patient education, and collaboration with physicians. Strong clinical knowledge in hospice and Medicare/Medicaid rules is required, with at least one year
