Job description
Gentiva is seeking an Afterhours RN Care Manager to provide compassionate hospice care outside standard hours, delivering direct patient care, coordinating after-hours admissions, and ensuring primary clinical oversight for assigned patients. You will work with the Interdisciplinary Group, complete timely documentation, educate families, and support workflow audits.
Salary is $83-92k with a comprehensive benefits package.
