Job description
Circadia Health is seeking a Nurse Practitioner or Physician Assistant to join our SNF and Home Health AI Care teams in Michigan. You’ll see patients first, act on real-time predictive signals, and coach facility staff to intervene earlier and reduce readmissions.
You’ll blend direct patient care with bedside tech integration, training staff, and participating in live demos and shadowed assessments across multiple sites within a 30–40 mile radius.
