Job description
Adaptive Home Health in Detroit is seeking a Registered Nurse (RN) Case Manager to deliver high-quality home health care across our field-based network. You’ll perform comprehensive in-home assessments, document care plans, and coordinate with physicians to manage patient outcomes.
You will lead a pod of LPNs and HHAs, ensuring OASIS/compliant documentation, quality care, and strong relationships with discharge planners and physicians.
