Job description

Adaptive Home Health is seeking a Registered Nurse (RN) Case Manager for field-based, full-time work in Detroit area. You will conduct in-home assessments, document OASIS/SOC/recert, and manage high-acuity patient care including wound care.

Lead care plans with physicians and supervise LPNs and HHAs, while building strong relationships with referrals. Join a team aiming to deliver exceptional patient care through AI-native tools, optimized routing, and streamlined charting to reduce after-hours
