Certified Home Health Aide (HHA)
Join a compassionate healthcare team as a Certified Home Health Aide (HHA) providing direct patient care and supportive personal assistance under the guidance of licensed nursing staff. In this role, you will deliver care aligned with individualized patient plans, ensuring comfort and dignity for patients ranging from newborns to the elderly.
Essential Responsibilities:
Perform duties within your certified scope of practice, demonstrating the knowledge and skills required by departmental policies and competencies. Accurately assess patient needs and contribute to the development, implementation, and review of comprehensive care plans. Monitor and report physical, emotional, and psychosocial conditions to the hospice team promptly, including symptom and pain management and end-of-life signs when appropriate. Maintain timely and precise documentation in accordance with organizational policies and uphold privacy regulations concerning Protected Health Information (PHI). Foster effective communication and professional relationships with patients, families, healthcare team members, and external agencies.
Education and Experience:
Successful completion of an approved Home Health Aide program within the region.
Licenses and Certifications:
Current certification as a Home Health Aide (HHA) in the applicable state. If hired as a CNA, HHA certification must be obtained within three months. Valid BLS/CPR certification from an approved provider within 30 days of hire or by the end of orientation. Driver's license and auto insurance meeting regional requirements if the position involves driving.
Preferred Qualifications:
Previous experience as a CNA or HHA, especially in end-of-life care.
Skills and Abilities:
Basic computer proficiency, strong multitasking and prioritization skills, effective communication, and the ability to apply patient care principles.
Physical Requirements:
Ability to perform heavy work occasionally exerting up to 100 lbs and frequently up to 20 lbs. Expect to stand or walk 51-75% of the workday.
