Job description
Sentara Norfolk General Hospital has a full-time APP to join Sentara's Case Management Pharmacy Program, VA . Work from home or at the hospital! Remote work available. Our Advanced Practice Providers (APPs) are essential to this foundation, and we seek to continue to grow their impact and scope of practice throughout our hospitals.
Position Highlights:
Join an experienced team of 1 MD, 9 nurse case managers, 5 clinical pharmacists, and 1 nurse supervisor
Primary Location: Sentara Norfolk General Hospital
Schedule: Mon - Fri, 8:30AM - 5:00PM, no weekends or holidays and working from home is available
Duties: Perform health risk assessments, re-write prescriptions
Review 150 patients per month
Work with Sentara Medicaid only
1 year experience required as APP in Family Medicine
Experience in Rheumatology or Infectious Disease preferred
