Hospice/Medical - Clinical Social Worker
Location: Albany, Oregon
Schedule: 8:00AM - 5:00PM x 5
Term: 13-week assignment with the potential for extension
Pay Rate: $48 - $52/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a LCSW for a temporary assignment in Albany, Oregon. These positions are 13-week contracts with the ability to extend.
Responsibilities for a LCSW:
Provide social work services to hospice patients and family members through the final stages of illness
Deliver services in home settings including patient residences, skilled nursing facilities, assisted living facilities, and adult foster homes
Provide medical social services and counseling support to patients and families
Offer spiritual support and bereavement support services
Collaborate with interdisciplinary hospice team to support pain and symptom management
Maintain current Healthcare Provider Level BLS certification
Maintain valid Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure
Utilize personal automobile for patient visits and maintain a valid driver's license
Maintain auto liability/property damage insurance as required by law and keep personal vehicle in good working order
Apply experience in hospital or medical social work settings to hospice care delivery
Work toward or maintain Oregon LCSW (Licensed Clinical Social Worker) licensure as preferred qualification
