Astrya Global, a San Diego based medical staffing agency, is hiring a PHARM for a permanent assignment in Milford, Delaware.
Responsibilities for a PHARM:
Establish a collaborative practice program with oncologists within identified areas of need including supportive care measures for patients receiving chemotherapy on an inpatient and outpatient basis
Conduct staff development programs or educational services to pharmacy staff, nursing staff, oncologists, and other healthcare professionals
Serve as a subject matter expert in oncology, completing drug information questions for the oncology service line and reviewing Electronic Health Record care plans to ensure consistency with current national guidelines
Round with Oncology Fellows, Nurse Practitioners, and Physicians to optimize therapy according to established national evidence-based guidelines, including recommendations regarding correct medications, monitoring outcomes, dose adjustments, and supportive care measures
Provide patient-specific medication education using the teach-back method for new oral oncolytics and intravenous chemotherapy as needed
Review oncology care plans within the Electronic Health Record, including new care plans and continual review of existing care plans for updates and appropriateness
Identify and evaluate medication-related problems based on clinical presentation, available history, or laboratory data
Serve as a preceptor to pharmacy residents and students
Actively participate in Oncology Service Line meetings to work towards improving safety, efficacy, and quality of cancer care
Promote and demonstrate rational, cost-effective drug therapy with a focus on cost avoidance and cost savings within the Oncology Service Line
Perform all other duties as assigned within the scope and range of job responsibilities
