Job description
UT Health San Antonio is seeking a LVN Case Manager in the Biggs Alzheimer’s Department to coordinate social services and case management within patient care, discharge, and home planning processes. You will liaise with healthcare providers, agencies, and families to ensure appropriate care and resources are in place.
The role requires strong nursing experience, knowledge of disease management, and proficiency in Microsoft Office.
