Job description

UT Health San Antonio is seeking a Licensed Vocational Nurse (LVN) Case Manager in the Biggs Alzheimer’s Department to coordinate social services and case management within patient care, discharge, and home planning processes.

The role requires 3 years of nursing experience and strong communication skills, with a focus on care continuity, utilization review, and advocacy for patients and families. On-site position within UT Health San Antonio system.
