Now Hiring: LVN & RN for Adult Hospice and Home Health

We are seeking a skilled LVN to deliver high‑quality nursing care in patients’ homes, under the supervision of a Registered Nurse. This role involves collaborating with the healthcare team, patients, and families to ensure the best outcomes.

Key Responsibilities
Provide direct nursing care to patients in their homes, ensuring the implementation of care plans in line with physicians' orders.
Regularly monitor vital signs, track medical conditions, document changes, and maintain accurate health records.
Monitor patients’ medication usage and report results, ensuring proper administration and dosage.
Work closely with the interdisciplinary team, patients, and families to assess, implement, and evaluate care plans.
Assist with emotional, personal, and medical support for patients, providing holistic care to meet their needs.
Evaluate patient behavior, diagnose conditions, and consult with healthcare professionals to improve patient outcomes.
Working Conditions
Work in patient homes, often in varying environmental conditions (including possible exposure to bodily fluids and infectious diseases).
Ability to travel locally to patient homes and manage flexible work hours.
Occasional emergency PRN calls and exposure to adverse weather.

Qualifications
Education: Graduate of an accredited school of vocational nursing.
License/Certifications Current state LVN / RN License.
BLS Certification (required).
Valid Driver’s License.
Experience: Minimum of 2 years as an LVN/RN in clinical, hospice, or home health settings (preferred).
