Job description
Supplemental Health Care is seeking a travel Home Health Physical Therapist for a 16-week assignment in White River Junction, Vermont. You will provide patient-centered PT care in home settings, conduct assessments, and develop individualized plans within a multidisciplinary team.
The role requires an active PT license, BLS, and 1+ year of home health experience with EMR proficiency. Expect 40 hours per week on dayshift, with travel between patient homes and adherence to Medicare/OASIS-E
