Job description
Atlantic Health System is seeking a Per Diem Clinical Pharmacist focused on ambulatory cardiology at the Overlook Medical Center in Summit, NJ. You will conduct medication histories, reconcile therapies, and optimize regimens for high-risk patients, collaborating with cardiologists and staff to ensure continuity of care.
You will educate patients, support transitions of care, and contribute to quality improvement projects within the ambulatory cardiology setting.
