Job description
WakeMed Ambulatory Care Nurse (ACN) is a Registered Nurse role dedicated to managing patient care in community settings. The ACN collaborates with physicians to identify at-risk populations and coordinates the plan of care with the patient, family, and care team.
The ACN supports PFCC and Nursing Practice standards, contributes to community health, and aligns with WakeMed’s mission. Strong nursing experience and NC licensure are required.
