Job description
WakeMed Health & Hospitals in North Carolina seeks an Ambulatory Care Nurse (ACN), a Registered Nurse responsible for providing nursing care management to assigned populations and coordinating care to help patients remain in their community setting.
In collaboration with physicians, the ACN identifies at risk patient groups, serves as a patient/family advocate, and communicates with the care team and resources. The ACN supports PFCC and nursing practice standards across WakeMed.
