Job description

Trinity Health Medical Group in Grand Rapids, MI, is seeking a part-time Clinical Pharmacist, Ambulatory Care, to provide outpatient drug therapy management and contribute to disease management programs within the PCMH offices.

You will participate in anticoagulation management, team-based care, and quality initiatives while supporting education and training for residents and students in a multidisciplinary health system.
