Job description
The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting. The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting. When appropriate, pharmacists who provide ambulatory care services may perform patient assessments, participate in collaborative practice agreements to provide medication management, provide collaborative drug therapy management, order, interpret, monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease, provide education to patients and caregivers, and document care plans in the medical record.

As the Ambulatory Clinical Pharmacist for the Neurology Oncology Clinic , you'll be at the forefront of transforming outpatient cancer care through precision medication management and seamless care transitions.
What You'll Do:
Deliver expert medication therapy management for patients with complex neurologic malignancies in an ambulatory setting.
Collaborate closely with neuro-oncology physicians, infusion center teams, and specialty pharmacy to optimize treatment outcomes.
Support advanced therapies including oral oncolytics, immunotherapies, and investigational agents.
Drive continuity of care across inpatient, outpatient, and infusion services-ensuring safe transitions and improved patient experience.
Enhance UVA prescription capture through proactive engagement with UVA specialty and retail pharmacy services.
Educate and mentor pharmacy learners and contribute to continuous professional development across the care team.

Minimum Qualifications Education: Bachelor's in Pharmacy or Pharm. D Degree required.
Experience: PGY-1 residency required or equivalent hospital experience. (3.years). PGY-2 residency is preferred for specialty areas.
Licensure/Certifications: Commonwealth of Virginia State Board of Pharmacy. License required. American Heart Association (AHA) Health Care Provider. BLS certification required.
Physical Demands - Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull Onboarding Requirements
Background checks and pre-employment health screenings will be conducted on all new hires prior to employment. All safety-sensitive positions with direct patient interaction also require a drug screen.
The pay range for this role is $64.37 - $89.07 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
