Job description
St. Mary’s Hospital - Kankakee is seeking an Ambulatory Clinical Pharmacist to provide patient care and advanced clinical pharmacy services in ambulatory settings, including referral-based HOPD pharmacist managed clinics, specialty pharmacy, and population health initiatives.
You will collaborate with prescribers, manage medication therapy, and document interactions across EMR platforms with strong communication and customer service skills.
