Job description
Vancouver Clinic seeks a Clinical Pharmacist Specialist for ambulatory care to optimize medications and lead population health initiatives. You will work with physicians and APCs to provide direct patient care in a collaborative, value-based care environment, including telehealth and in-person visits.
