Job description
Kansas Health System is seeking a PRN Ambulatory Clinic RN Care Coordinator to support the Heart Failure Device Team at Bell Hospital. The role focuses on coordinating care for patients with advanced heart failure and implantable devices, reviewing device data, and providing patient education.
The RN will work with providers, patients, and families to ensure timely interventions, optimize outcomes, and maintain continuity of care in the ambulatory setting.
