Job description
Parkview Health in Fort Wayne, IN seeks a registered nurse to lead care coordination using remote patient monitoring. You will assess and manage patients’ primary and social needs, coordinate with hospital and clinic teams, and support transitions of care to improve outcomes.
Responsibilities include monitoring vitals via RPM, coaching patients, and ensuring adherence to care plans and payer requirements. Indiana RN licensure and BLS are required.
