Job description
Provides direct nursing care in an ambulatory setting, coordinating patient care with physicians and other disciplines. Assesses patient needs, implements and updates individualized care plans, monitors and communicates patient conditions, carries out clinical orders, and supports discharge planning. Requires a current Connecticut RN license, BLS certification and cardiology RN experience.
