Job description
Provides direct nursing care in an ambulatory setting, coordinating patient care with physicians and the interdisciplinary team. Assesses patients, develops and revises individualized care plans, monitors and communicates patient conditions, carries out orders, and supports discharge planning. Requires a Connecticut RN license, BLS certification, and cardiology RN experience.
