Coverage Dates ASAP, pending credentialing 13 weeks with an option to extend
Shift Schedule Details & Shift Duration: OR Rooms Multiple Shifts 7a-5p 10 rooms 5 rooms from 5p-7p 3 rooms until midnight; Folsom 6 rooms from 7a-5p - Client wants 2 providers covering Sat, Sun, Mon, Wed for 6 months

Case types: B&B, General Surgery, Ortho, GYN, Podiatry, ENT, Robotics, HNS
• Heads: Some Head/Neck/Spine Cases but will try to limit but providers have to be able to take, if needed.

REQUIREMENTS:
• What is the minimum length of commitment requirement per month?
• Certifications/Qualifications Required (ACLS, BLS, MSQA, TEE, etc.): BLS, ACLS
• Board Certified Required: Yes
If BC is not required, Number of Years of Board Eligibility Allowed since Residency/Fellowship Completion: BC or truly BE
• IMLC State: No
