Job description
Northwell Health seeks an Aortic Center Nurse Navigator, an RN coordinating complex care for patients with aortic diseases. You guide patients through pre-procedure, hospitalization, discharge, and follow-up, coordinating with multidisciplinary teams for seamless care.
The role reports to the Aortic Center Program Director. Requires RN licensure in the state and 3+ years of nursing experience; BSN preferred, vascular surgery exposure and EMR familiarity are valued assets.
