Position Type: Full-time and Part-time opportunities
Lacks Employee Health Facility operated by Trinity Health for patients and their families enrolled in the Lacks Medical Plan

Lacks Employee Health Facility is located at nd St SE Kentwood, MI 49512
The provider will dedicate 100% of their time to practicing at the newly renovated Lacks Employee Health Facility, which features spacious modernized patient rooms.
Clinic is a concierge type practice with 30-minute patient visits offering full service primary care, well visits and chronic disease with low-cost acute care visits as well as occupational health
On-site medications for patients and lab draws
Current clinical team includes 1 physician, 1 advanced practice professional, 5 support staff (mix of MAs & BOC), and a dedicated practice leader.
Provider will be able to deliver high level patient centered care, fostering strong, trusting relationships while effectively managing a shared patient panel of approximately 1,500 individuals across providers.
Clinic is open 7a-5p Monday - Friday, no weekends or holidays
No on call

Requirements
Provider experience required
Board certified prior to employment
Active Michigan medical license (or ability to obtain)
DEA license
Duties & Responsibilities
A typical day may include:
Providing direct patient care
Collaborating with multidisciplinary teams
Participating in teaching and mentoring
Documenting in Epic EMR
Engaging in quality improvement initiatives
Supporting community health efforts
