Job description
Hospice of the Bluegrass Inc. seeks an APRN to exemplify advanced nursing practice, provide direct participant care, and evaluate outcomes within the PACE Center model.
The role involves collaboration with the Chief Medical Officer, Medical Director, and referral sources to establish individualized plans of care, in compliance with nursing regulations and organizational policies. CPR certification must be maintained.
