Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home physical therapy, assess patients, and develop rehabilitation plans aligned with physician goals. You will train and supervise PTAs and Home Health Aides, document progress, and coordinate with physicians and families to optimize recovery.
The role includes 40 scheduled hours per week, patient-facing duties, and adherence to Humana's TB and driver safety programs.
