Job description
Care Initiatives Hospice is seeking a compassionate RN Case Manager to provide in-home and community-based care for patients in Council Bluffs and surrounding areas. You will perform initial assessments, develop and update plans of care, coordinate with physicians and interdisciplinary teams, and deliver education to patients, families, and caregivers with dignity and respect.
