Job description
Optimal Care - Evergreen is hiring a Home Health Physical Therapist in Bingham Farms, MI. You will deliver one-on-one rehab in patients’ homes, assessing function, mobility and safety to restore independence after surgery, injury, or illness.
You’ll design individualized plans, educate patients and caregivers, and work with an interdisciplinary team to achieve measurable progress and improved quality of life.
