Job description
The VIOS Clinic is seeking a Registered Nurse (RN) Home Health Case Manager in Missouri to oversee patient care in a home health setting. You will manage caseloads, supervise LPN visits, coordinate with therapy teams, conduct assessments, and develop personalized care plans for conditions such as CHF, COPD, diabetes, wounds, and medication management.
