Job description
VitalCaring is seeking a Field-Based Home Health RN to lead care plans in patients' homes across an assigned territory. You will assess, coordinate, and drive outcomes with a collaborative clinical team, delivering skilled nursing care and education to promote independence.
You will have autonomy to make real-time clinical decisions, manage complex caseloads, and participate in on-call rotations while maintaining high-quality documentation and patient relationships.
