Job description
VitalCaring is seeking a Field-Based Home Health RN to lead the care plan for patients in their homes across an assigned territory in Florida. You will assess, plan, and deliver skilled nursing care with a focus on reducing hospitalizations and promoting independence.
You will collaborate with physicians, therapists, and care teams with strong autonomy supported by a clinical leadership team. A valid RN license, CPR, and 1+ year of home health or acute care experience are required.
