Job description
VitalCaring is seeking an experienced Home Health Registered Nurse to lead the plan of care for patients in their homes. You will coordinate across disciplines, drive outcomes, and deliver skilled nursing care from admission to discharge.
Responsibilities include comprehensive in-home assessments, individualized plans, and case management with strong clinical judgment. Expected collaboration with physicians and care teams, EMR documentation, and on-call participation.
