Job description
Empatia, part of Gentiva, is seeking a compassionate Palliative Care Nurse Practitioner to provide advanced palliative care at patients' homes, collaborating with the Palliative Medical Director to enhance quality of life and manage complex needs.
You will work autonomously within an interdisciplinary team, performing assessments, developing care plans, adjusting treatments, and guiding patients and families through care options and transitions.
