Job description
21st Century Home Health Services (21HHS) is seeking a Full-Time Physical Therapist Case Manager, Level 2, based in Fairfield, CA. You will evaluate patients in their homes, develop care plans, and supervise PTAs to restore function and promote independence.
Responsibilities include coordinating with physicians, documenting progress, and managing a targeted territory with flexible hours. CA license, BLS/CPR, and driver’s license are required.
