Job description
21st Century Home Health Services (21HHS) is seeking a Full Time Registered Nurse Case Manager, Level 2 for the Novato area. You will create, implement and execute the plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
You will assess patients/families, develop advanced care plans, and collaborate with an interdisciplinary team to deliver high-quality care. Flexible schedules and opportunities for advancement are provided.
