Job description
RVNAhealth in Connecticut is seeking a compassionate RN Case Manager to join our Home Health team serving Fairfield and Litchfield Counties. You will coordinate care from SOC through discharge, partnering with physicians, therapists, social workers, and caregivers to provide high-quality, in-home nursing services.
Requirements include an accredited nursing degree, CT or Compact RN license, at least one year of nursing experience, and willingness to travel within the assigned territory.
