Required clinical activity in the last 24 months (case logs) Applicant must provide documentation of provision of hematology/medical oncology services representative of the scope and complexity of the privileges requested during the previous 24 months (waived for applicants who completed training during the previous year).
Will provider work solo? Will work 1:1 with a Med Oncologist
Licensed Candidate: Yes, active WA license in hand at time of present
Required certifications (Must be from American Heart Association or American Red Cross as applicable) BLS
EMR EPIC Proficient user
