Job Description:
- Join a dynamic healthcare environment!
- Engage with a culturally rich community located in the stunning Colorado Plateau.
- Deliver services across a 4,400 square mile area, serving diverse populations including the Navajo, Hopi, and Southern Paiute tribes.
- Empower high-risk patients as an Outpatient/Inpatient Social Worker.
- Design and manage a continuum of care to help clients achieve self-sufficiency and positive outcomes.
Key Responsibilities:
- Conduct assessments and develop care plans tailored to individual client needs.
- Coordinate services and advocate for clients, ensuring they receive necessary resources.
- Collaborate with a multidisciplinary team to monitor patient progress and adjust care plans as needed.
- Evaluate patients in the Emergency Department for care coordination and medical necessity.
- Propose and implement outpatient plans for safe discharges with physician approval.
- Engage with individuals facing mental health challenges to support their well-being.
Necessary Qualifications:
- Education:
- Master's degree in social work (MSW).
- License/Certification:
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with valid and unrestricted licensure.
- Current Basic Life Support (BLS) certification by the American Heart Association.
- Experience:
- Minimum of three (3) years in a clinical care setting.
- Knowledge of discharge planning, case management, and utilization review processes.
- Proficiency in electronic health record systems.
Shift Information:
- Monday - Friday: 8 AM - 5 PM.
- Callback on weekends and holidays required.
