nnFacility: Ascension Alexian Brothers Behavioral Health

nnDepartment: Inpatient Unit
n
nnSchedule: Day shift | PRN
n
n nHow you’ll make an impact in this role
nnnOversee primary needs of the patient, including assessment of the client and family/caregivers, and develop a comprehensive plan of care.
n
nnProvide primary home care/consultation to complex client and family needs within area of expertise and in collaboration with physician.
n
nnMonitor patient care through assessments, evaluations and/or patient records.
n
nnMaintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
n
n nWhat minimum requirements you’ll need
nLicensure / Certification / Registration:
nnnCounselor credentialed from the Illinois Department of Financial and Professional Regulation preferred.
n
nnSocial Worker credentialed from the Illinois Department of Financial and Professional Regulation preferred.
n
nnCase Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
n
n nEducation:
nnHigh School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
n nAdditional Preferences:
nnnMaster's Degree in Counseling or Social Work
n
nnExperience in Case Management and Discharge Planning highly preferred
