Job description
Family Health Services is seeking a Behavioral Health Consultant to join our primary care clinics. The BHC provides brief behavioral interventions, case management, triage, and referrals to support patients with chronic illness and behavioral concerns within an integrated team-based model.
The role emphasizes collaboration with care teams, documentation in EHR, and participation in performance improvement activities. A master’s degree and state licensure in Idaho are required.
