Job description

Westpark Springs, a mental health and addiction treatment center in Richmond, TX, seeks a PRN Discharge Coordinator to support patients after discharge. You will coordinate aftercare plans, assess needs with the care team, and ensure smooth transition with comprehensive resources and timely documentation.

Collaborating with physicians, nursing, and case management, you’ll develop detailed discharge summaries, maintain resources, and address crises while upholding HIPAA and regulatory standards.
