Job description

Aizer Health in New York is seeking a Licensed Practical Nurse to join our behavioral health team. The role focuses on comprehensive patient assessment, administration of medications (including long-acting injectables), blood draws, and coordination of care across psychiatry, primary care, and case management.

The LPN will educate patients and families, ensure accurate documentation in the EHR, and participate in quality improvement while upholding HIPAA and regulatory standards.
